Let's Talk Testosterone
There is a lot of chat about the benefits or not of adding testosterone to HRT. Testosterone is produced by women with levels gradually reducing with age, rather than specifically falling during the menopause transition, unless ovaries are removed, when testosterone production falls by about 50%. Even in that situation, testosterone does not necessarily need to be replaced.
Testosterone is currently only recommended along with HRT for women with persistent low sex drive, after all other contributory factors (and there are many!), have been addressed. For example, many women experience vaginal dryness and discomfort which understandably affects interest in sex. This can usually be easily treated with the use of vaginal estrogen, of which there are many types, along with moisturisers and lubricants.
If testosterone is used after full discussion and consideration, it should be used as a trial in the first instance. If there is no benefit after 6 months, it should be stopped.
Benefits for fatigue, brain fog, and mental health are often described, but evidence so far has not demonstrated such benefits. More research is underway.
